ETD Forms

To submit an ETD you will need to fill out all the usual university forms required for all dissertations, with the possible exception of the ones in the UMI booklet, Publishing Your Dissertation. See item 8 on the ETD Guidelines for details.

Electronic Dissertations Request for Delay of Posting

Under certain circumstances you may request a publication delay (embargo) for your dissertation. To do so, download the form This is a link to a Portable Document Format (PDF) file, fill it out, and get the necessary signatures before submitting your dissertation to the OhioLINK ETD Center.

Online Submission Form

When your dissertation has been approved, the next step is to access the OhioLINK ETD Center online submission form This is a link to another web site which will enable you to upload the document.

ProQuest/UMI Forms and Information Packet

ProQuest/UMI has a 26-page informational packet with some forms you may need to submit to your College or School office. You can download and print the specific pages you might need as outlined here:

  • Copyright Registration Form This is a link to a Portable Document Format (PDF) file (needed if you want ProQuest/UMI to register your copyright; must be accompanied by $65 payment)
  • Author Discount Offer This is a link to a Portable Document Format (PDF) file (needed if you want to order printed copies of your dissertation at a discount; must be accompanied by payment)
  • Sample Permission Letter This is a link to a Portable Document Format (PDF) file (useful for obtaining permission to use co-authored or previously copyrighted material in your dissertation; permission is required for each such item)

The information is generally useful, though some recommendations are not in line with KSU policies (which take precedence). It includes the ProQuest/UMI Publishing Agreement which is effectively incorporated in the OhioLINK ETD Center online submission form.
