Online Reserve Request System - Teaching Faculty Use Only

Teaching faculty only may use the UL Personal Login to access the online request system for Reserves.

Reserve Request Forms (PDF Format) - Faculty and Staff Use

Forms are available in print format in the Reserve Services office.

A. Course Information, List, and Copyright Forms:

  • Course INFORMATION Form This is a link to a Portable Document Format (PDF) file
    This form collects required information about the course including course instructor(s), course name and number, cross-listings, and student enrollment.
  • Course LIST Form This is a link to a Portable Document Format (PDF) file
    This form is used for listing physical, print materials (library and/or personal copies of books, maps, DVDs, etc.). Do not include journal articles or book chapters on this form (use the Copyright Request Form, below). Submitting this form along with the materials will expedite processing and availability. This form must be submitted with the Reserve Services Course Information Form (above).
  • COPYRIGHT Request Form This is a link to a Portable Document Format (PDF) file
    This form is used for journal articles, book chapters, lecture note sets, sample exams, etc. One form must be completed per article. Detailed bibliographic and enrollment information is needed to request copyright permissions from publishers. Reserve Services exercises the right to refuse to accept a copying order if, in its judgment, fulfillment of the order would violate copyright law. Reserve Services cannot be held responsible or liable for mutilation or theft of instructors' personal materials. Reserves Services takes preventative measures against theft and mutilation of materials placed on reserve, which include barcoding, reserve tapes, and security measures that activate the Library's gate alarm system.

C.  Permission for Faculty Use of Student Materials for Reserve This is a link to a Portable Document Format (PDF) file

According to Kent State University Policy, students own the copyright to works created as part of their coursework.  Faculty using student-created materials for electronic or print Reserve for more than one semester must have this waiver form completed and signed by both student and faculty.  Signed waivers will be kept on file in the Reserve Services office.  One form per student is required; however, multiple works by one student may be listed on one form.
