Reserve Forms For Faculty And Staff Use

A.  Course INFORMATION and LIST Forms

1.  Reserve Services Course INFORMATION Form At Kent State University This is a link to a Portable Document Format (PDF) file
Reserve Services Course Information Form has to be completed to facilitate reserve request and accurately process information for the instructor and course.

2.  Reserve Services Course LIST Form – Kent Main Library This is a link to a Portable Document Format (PDF) file
Reserve Services Course LIST Form is used for listing "print" materials and how each material is to be placed on reserve for the course. "Print" materials include library books, personal copies of books, maps, compact discs, etc. Submitted form (preferably with materials) will expedite processing and availability of materials on reserve. Also, this form must be submitted with the Reserve Services Course Information Form At KSU.
Note: Please do not include journal articles or book chapters on the List form. These materials will require a copyright request form, instead of listing them on the List Form (see below for Reserve Services Copyright Request Form).

Bibliographic information for articles from journals and books is needed for Reserve Services to request copyright permissions from publishers. Permission must be obtained, or royalties paid, for materials retained on reserve for longer than one semester. Faculty members are notified of a publisher's reply if permission is denied or if special conditions are required, such as fees. For more information, please see the Library Reserves Policy.

The copyright law of the United States (Title 17, United States Code) governs the making of photocopies or other reproductions of copyrighted materials. Libraries and archives are authorized to furnish a photocopy or other reproduction of materials under certain conditions specified in the law. One of these specified conditions is that the photocopy or reproduction is not to be "used for any purpose other than private study, scholarship, or research." Furthermore, if a user makes a request for, or later uses, a photocopy or reproduction for purposes in excess of the "Fair Use" clause, that user may be liable for copyright infringement. Fair Use means that only one instance of the journal article, or 10% or less from a book, may be copied.

Reserves Services exercises the right to refuse to accept a copying order if, in its judgment, fulfillment of the order would violate the provisions of the copyright law.

Reserve Services cannot be held responsible or liable for mutilation and/or theft of instructors' personal materials. Reserves Services takes preventative measures against theft and/or mutilation of materials placed on reserve, which include bar coding, reserve tapes, and security measures that activate the Library's gate alarm system.

According to Kent State University Policy, students own the copyright to works created as part of their coursework.  Instructors using student-created materials for reserve (electronic or print reserves) must have this waiver form completed and signed by both student and instructor.  Signed waivers will be kept on file in the Copyright Clearance Services office.  One for per student is required.  Multiple works by one student may be listed on one form.
