InfoExpress Authorization Form for On-Campus Customers

By submitting this form, I authorize InfoExpress staff to charge $3 per photocopied article to my KentLINK account (library "fine" account) or to my departmental or grant account number at the end of each semester. Accrued charges of $25 or more for faculty/staff (or $10 or more for students) on my KentLINK account will result in suspension of borrowing privileges until payment is received. I certify that I am authorized by my department to use the Departmental Account Number I specify and will be responsible for any charges if my Department refuses payment. If I am a Library Representative, I receive $100 in free photocopying services per fiscal year and agree to pay $3 for each photocopied item after $100 has been met.
