Kent State University Libraries has made it possible for current faculty, staff, students, retired faculty or staff, and emeriti (the “patron”) to designate another individual (the “proxy”) to request, pick up, or renew material in the patron’s name. Acceptable proxies are faculty research or teaching assistants, members of the patron’s immediate family (e.g., spouse, sibling, parent), or another faculty member.

By completing and signing this form, the patron agrees that all material requested and/or borrowed by the designated proxy remains the responsibility of the patron, including any fees or fines that may be incurred. The patron also agrees that it is their responsibility to monitor how their account is being used by their designated proxy. Please read the Proxy Borrowing Policy.

A Proxy Borrowing Agreement Form must be filled out for each individual proxy. A total of four (4) proxies may be assigned to one (1) patron. The patron/proxy agreement expires one (1) year from the date of activation.
